After action report: first KOM went well, then I really struggled. Both hamstrings cramped really bad, first time ever, I drank enough water and sodium, only lost 3 pounds of water (1.5% BW), so fatigue was the issue from stochastic efforts. The last hour of the ride, my heart was beating out of my chest, lungs hurt breathing at any effort, and was super lightheaded. ears are also popping? 350mg of caffeine was probably too much in hindsight 

Things that went well: Solid taper, carb/nitrate/sodium loading, on the bike nutrition and good rest this week

Also not to self diagnose, but I've probably been anemic for a while. Ferritin just tested in at 30, when the acceptable lower bound is 38
